Could my daughter get into graduate school for social work with a 2.5 undergrad GPA?

My daughter did poorly in college because she is suffering from Asperger's syndrome and thyroid problems. How could she explain her disability to the graduate admissions officers? My daughter graduated from Hunter College last year with a Bachelors degree in Psychology. Now, she is looking for any MSW program in New York City.
